The CH2 form is used to make a claim for Child Benefit for up to two children.
You can complete a CH2 form once the birth of your child has been registered, or once they come to live with you.
Once you have completed the CH2 you should submit it to the Child Benefit Office, using the address on the form.
You don’t need to send your child’s birth certificate with your claim. You will need to send an adoption certificate if you have adopted your child, however.
